Data Quality and 18 Week Monitoring Team

The Data Quality/18 Week Team is lead by Katy Graham, Patient Access Facilitator. The team records all referrals into the trust onto hospital systems and ensures that the information we hold on our systems is accurate. They also monitor patients as they progress on the 18 week pathway and will alert the appropriate service manager to delays along the patient pathway ensuring that wherever possible patients receive their first definitive treatment within 18 weeks.
