The Time to go Home campaign was launched in October 2015 with a range of initiatives that focus on discharging patients in a timely but safe manner and avoiding unnecessary admissions.

Throughout the campaign we have held a range of listening events with staff and as we move forward the campaign will build on the successes of the work that has already gone on and start to tackle some of the issues that you have told us are important to you.

The campaign team are currently working with staff to develop a range of new and existing patient focussed initiatives that support you and your patients.

The initial focus will be on supporting safe, effective discharge through:

  • Developing ward based MDT board rounds
  • Embedding the principles of meaningful EDD and criteria led discharge
  • Supporting our patients and staff with information relating to discharge processes
  • Appropriate referrals for assessment and ongoing care needs
  • Working with partners in the Third Sector and care homes
  • Improving access to pharmacy information

This page will provide quick access to campaign information and will be updated as new initiatives are developed through the year.
